In a small saucepan combine vanilla caramels and whipping cream. If desired, stir in chopped pecans. Cook and stir over medium heat until caramels melt and mixture is smooth. Set aside to cool slightly.
Meanwhile, in a small bowl combine peanut butter and marshmallow creme. Spread peanut butter mixture over flat sides of 10 cookies. Place cookies on foil.
Using a spoon, drizzle about 2 teaspoons of the cooled caramel mixture evenly over peanut butter mixture. Top each drizzled cookie with another peanut butter cookie, flat side down. Drizzle tops of sandwich cookies with melted milk chocolate. Let stand until set.
